Dylan Mulvaney may have risen to prominence as an actress on social media for her series Days of Girlhood but now, she’s more of a social media influencer who is known for documenting her gender transition in TikTok and detailing everything about it in her daily videos. She has been doing it since early 2022 and by now, she has garnered more than 10 million followers on TikTok.

Dylan Mulvaney broke her social media hiatus at the beginning of 2023 to unveil her new look after she underwent facial feminization surgery. She had been quiet on social media since December 21 while she was recovering from the plastic surgery. After she fully recovered from the procedure, she returned to social media and the first thing she did was reveal the results of the procedure by letting her fans in on her new look in the most high-fashion way possible.
Dylan Mulvaney posted a video to her Instagram account titled The Face Reveal in which she is serving Old Hollywood glamour and futuristic fashion and also her fresh face after plastic surgery. The video captioned ‘Facial feminization part 1’ features the TikToker in two different outfits dancing and posing as orchestral music plays in the background. She also shared several black-and-white photos of her from a photo shoot in a follow-up post. In the video, she said,
That’s a wrap on face reveal! Oh my gosh, hi! I missed you! You know I have a flair for the dramatics, but it’s so good, right? I’m so happy, and it’s still me; it’s just a little bit softer of a version, and I just hope that all trans and nonbinary people can get the gender-affirming resources that they need because this is life-changing and sometimes life-saving.

Dylan Mulvaney also made a TikTok where she detailed all the plastic surgery procedures she underwent for facial feminization and shared what happened during her operation and the subsequent recovery process. In the video she posted on January 30, she said,
The surgery itself took multiple hours. It was the first time I was ever under anesthesia and I don’t remember a thing. I had a hairline advancement, a brow bone shave, a rhinoplasty, minor cheek enhancement, a little lip lift, a jaw shave, a chin reduction and a tracheal shave.
Dylan Mulvaney also noted that facial feminization was sort of like choosing your own adventure because not all surgical plans were the same and included the same plastic surgery procedures and patients can pick the areas they would like to have worked on. She explained that you can get any procedures or not at all, depending on what you have dysphoria over.
Dylan Mulvaney shared that it can take up to an entire year to recover from facial feminization surgery.

And regarding the healing process, Dylan Mulvaney said that she had “some of the most insane swelling she had ever experienced in her entire life” by day two, three, and four as if getting sent home bandaged up with pain medicine and an order to not eat anything solid for a week was not enough. She got every plastic surgery procedure done in one fell swoop because she knew about the recovery process that it was a lot. As she said, it can take up to an entire year to actually see all the swelling gone.
Reflecting on plastic surgery for facial feminization overall, Dylan Mulvaney said that it needs to be looked at in a different light and should be considered important as other gender-affirming health care.
This is an extremely privileged procedure and I want to stress that is not just typical plastic surgery. Had I not been trans, these wouldn’t have been changes that I would have sought out. But because of my gender dysphoria, I felt I need it. And I want us to all start viewing FFS as just as important as other gender-affirming health care. Trans people spend years saving up for these type of procedures because they are rarely covered by insurance and it truly already brought me so much peace that I hope we can all start advocating for better trans health care.
